The Johnsburg Sanitary District Commission was in attendance on Tuesday, June 1st, 2010, at 3:45 p.m., for a Pre-Construction meeting. The purpose of the meeting was to discuss the Meyer Court sewer extension project with the contractors, sub-contractors and utility firms.
Members present were President Franz Schmitz and Commissioner Ken Schneider; Recording Secretary Brenda Schneider; Eric Otte, JE Arthur; Jason Meyer; and Rick Schneider, Town & Country Underground Utility Construction. Absent was Commissioner Dennis Lefeber.
Eric Otte, JE Arthur & Associates, planned and moderated the meeting.
Construction will begin as soon as the prevailing wage determination is received from the Wisconsin Dept. of Workforce Development.
Construction will be completed in one week.
J.E. Arthur will represent the District as its Resident Inspector. Arthur will inspect pipe laying, manhole construction, backfilling and testing and televising. Mandrel testing will be done upon completion. However, the televising is not to be completed for at least 30 days. Arthur does not believe it is necessary to be on site during the televising; however, Arthur will view the video and report to the District any abnormalities.
Utilities are yet to be installed in the construction area.
Jason Meyer will install check bales. It was noted that the construction area is less than one acre, therefore, an erosion control and stormwater management permit is not required.
Meyer has staked the manholes and laterals. He has modified the lateral locations from the plan in order to have just one cut across the road. Arthur will verify the stakings. Town & Country shall preserve stakes throughout construction operation. Meyer shall provide Arthur will fieldnotes, cutsheets, etc.
Rick Schneider shall be the Contractor Representative.
Meyer explained sewer location modifications he has requested due to the need to protect the road base. A utility easement is noted on the plan; however, Johnsburg Sanitary District is not the beneficiary of the easement. Arthur will determine if easements with the District are necessary due to the location modifications. Arthur and Town & Country will do a site visit to determine the adequacy of the modifications.
Meyer shall submit financial surety in the amount to be determined. Town & Country shall submit a certificate of insurance.
Prevailing wage rate determination is expected soon. The Department of Workforce Development determination shall be followed unless the Meyer has a contract with Town & Country dated prior to January 1, 2010.
Town & Country shall submit cutsheets of the materials to be used prior to construction.
Town & Country will restore the construction area to rough grade. Meyer shall do all final restoration.
Town & Country shall supply lien waivers after the project is complete along with lien waivers from each sub-contractor and supplier. Town & Country shall provide a list of subcontractors and suppliers prior to construction.
Brenda Schneider and Arthur will communicate regarding the receipt of the bond and certification of insurance.
June 1st, 2010 Johnsburg Sanitary District 2.
